Create a Personalized Memorial Page
Create a Personalized Memorial Page to share with families and friends over a secure platform to ask for their support in memory of a loved one. This page can be customized to include a photo, story about your loved one, and allows you to specify the area of care that you would like the donations to be directed to. You can choose to include the custom link within an obituary, and track and send personalized emails to those who have donated in memory through your page.

If you are interested in directing your donation to a specific hospital site or program that is meaningful to you, please contact us to learn more.
Sample Obituary Wording
If you are in need of sample obituary wording, we recommend:
In lieu of flowers, donations can be made to support the [insert program and/or hospital site] at Trillium Health Partners Foundation. Donate at trilliumgiving.ca/inmemory, by phone at (905) 848-7575 or by mail to Trillium Health Partners Foundation.
Donation cards provide guests an opportunity to give back in lieu of flowers. Guests can mail their donation to Trillium Health Partners Foundation using the addressed envelope provided with the donation card. If you would like donation cards at your ceremony we are happy to process your request.
Notifications When Donations are Received
If you would like to receive notifications of all In Memory donations received, please contact us. Notifications will include donors’ name and addresses, however individual amounts will remain confidential.
